Installation of New Service Connection


Schedule of Availability of Service:
     Day: Monday – Friday (except holidays) Time: 8:00 AM – 5:00 PM

Who May Avail of the Service:
     Any interested individual, firm or office.

What are the Requirements:
     1. Orientation upon filing of application
     2. Any of the following:
          a. Certificate of residency from the Barangay Captail
          b. Valid I.D. with picture.
          c. Current Residence Certificate
          d. Permits (Barangay, Municipal and Provincial), if needed
          e. Contact Number if any

Duration:
     5 Hours and 18 Minutes (excluding travel time and depending on the schedule of turn-over
                                          of investigated applications and schedule of orientation)
